Our Patient Admission Policy

• The patient or their representative must bring insurance cards along with a photo ID.
• The patient or their representative should have their chosen form of payment available to cover any co-pays or remaining balances.
• The patient or their representative needs to carefully verify all information listed in their chart, including names, addresses, and other demographic details.
• Patients should have arranged transportation home and ensure that a responsible adult is available to assist them in the evening. For the patient’s safety, we recommend a responsible adult remain with them at the center throughout the visit.
• Parents or legal guardians of pediatric patients (17 years old or younger) are required to stay at the facility until the patient is released. Legal guardians should bring any necessary legal or guardianship paperwork with them.
